How do we schedule our Fertility Consultation?

   

If you and your significant other are interested in scheduling an Immunitrition Nutritional Fertility Consultation, please call (877) 773-9229 or e-mail your consultation request to consultations@immunitrition.com.  You may also visit the "Contact" page of this Web site to make a fertility consultation inquiry.  Your first and last name, phone number and e-mail address are required to handle your inquiry properly.  Within 24 hours an Immunitrition representative will contact you to begin the fertility consultation scheduling process.      

 

 

 

Do we need to do anything to prepare for our Fertility Consultation?

 

Before your actual consultation takes place, Immunitrition will be supplying you with a special, four-part health questionnaire to better help analyze your health status, as well as the health status of your significant other.  These important documents will aid us in recommending the best solutions to apply to your unique fertility goals and needs.  Your consultation cannot take place without this necessary information.  Please fax, snail mail or e-mail your questionnaires back to us at least ONE WEEK before your Fertility Consultation date.  You may see some repeat questions throughout these questionnaires.  This is intentional and will help us to see the "over all" picture of your current health status and fertility concerns.  NOTE: Regarding the 3-Day Food Journal, you will need to print out three copies - one copy for each of the three days you will be documenting your food intake.  Please do the same for your significant other.    

 

Please click on the appropriate links below to download your FOUR-PART questionnaire.  We have provided each questionnaire in two formats - Adobe PDF and Microsoft WORD.  They will both display the same way on your computer screen.  The WORD format will allow you and your significant other to complete the questionnaires right on your computer using your Microsoft Word Program.  You can then save your questionnaire documents to your computer's hard drive and send them back to us via e-mail as an attachment.  This is the quickest way to expedite the return of your questionnaires to the Immunitrition clinic.

 

You may wish to print out the PDF versions of the questionnaires, fill in your information by hand, then return them to us via the U.S. postal system.  If so, please mail to the address listed on the last page of your "Confidential Health Questionnaire" .  You may also fax your completed questionnaire to (480) 393-5410 or e-mail to: consultations@immunitrition.com

If you would like for us to fax you these questionnaires, please include a fax number with your contact information.  If you prefer us to e-mail them to you, we will attach the documents in WORD format so that you may fill them out on your computer.  If you would like us to mail the questionnaires via the U.S. Postal Service, be sure to include your address with your contact information. 

 

Once your questionnaires have been completed and returned to us, a representative will contact you to schedule your Fertility Consultation.  Payment is due at the close of your consultation.  Don't forget to make a copy of your health questionnaires for your records before forwarding to us.  If you wish to include any tests such as blood analysis, mycology, digestive stool analysis, etc., please keep the originals and send copies of these tests along with your completed health questionnaires.         

 

 

 

Where will our Fertility Consultation take place?

 

If you are a New York resident living within two hours outside of New York City, your consultation will take place in Franklin Square, New York.  Caroline Barringer currently sees clients on weekdays and some weekend hours at her clinic located in the Extreme Wellness Center at Jayson Pharmacy.  Please do not call the wellness center/pharmacy to schedule a consultation.  Please call Immunitrition at 877-773-9229 to schedule your appointment.  The wellness center addresss is:

What other services are offered at the Immunitrition Kind Care Clinic?

 

The Extreme Wellness Center and Med Spa at Jayson Pharmacy offers a wide variety of health and spa services.  Immunitrition offers Nutritional Fertility Consultations, as well as ionic foot baths, far infrared saunas and hydro therapy massage to aid parents-to-be in dietary modifications and gentle detoxification protocols.  Other services offered at the center by fellow practitioners include anti-aging techniques such as facials, cellulite therapy, body wraps, hot stone therapy and more.  There is also a Nurse Practitioner, Lisa Del Pozo, on staff for those seeking care in both medical and holistic modalities.  Lisa is available by appointment only for family medical care including annual physicals, work physicals, yearly female examinations, blood tests (blood drawn on site by the nurse or a nationally certified Phlebotomist) and so much more.  To schedule an appointment with Nurse Lisa, please call her answering service at: (718) 496-9233.  Do not call the pharmacy.         

 

 

 

What kind of clothing should we wear to our Fertility Consultation?

 

Please wear comfortable clothing such as a t-shirt and sweat/stretch pants.  There will be a functional, hands-on assessment performed on you and your significant other during your Nutritional Fertility consultation, therefore tight clothing may not be the best choice for the most accurate testing or general comfort during your Fertility Consultation.      

 

 

 

Should we eat or drink before our Fertility Consultation?

 

We prefer that you and your significant other refrain from EATING at least two to three hours before your consultation, but we do hope that you do drink plenty of water to stay properly hydrated before you come in.  Proper hydration is essential to feeling healthy and looking younger!   

 

 

 

Should we bring anything along with us to our Fertility Consultation?

 

Yes.  Please bring any and all supplements/medications you and your significant other are currently taking (nutritionals, vitamins, homeopathic rememdies, herbals, prescription and non-prescritption drugs, etc.) as well as any tests either of you have recently taken such as clinical blood tests, hormone panels, digestive stool analysis, heavy metal panels, parasite panels, etc., etc.  You can bring all of your supplement/medication bottles in a shopping bag, basket or cardboard box.  Do not bring the original copies of your medical tests and panels.  Please make copies and bring these new copies to your consultation to keep in your client file.  All client files at the Immunitrition Clinic will be kept in a secure location (locked filing cabinet) for your privacy and protection.    

 

 

 

How do we pay for our Fertility Consultation?     

 

Immunitrition accepts all major credit cards - Visa, MasterCard, Amex and Discover.  Payment is due upon completion of your consultation.  You can call us with your credit card number at (877) 773-9229 or you can buy your consultation right at our ONLINE STORE.  If you prefer to send a personal/business check or money order, please make all checks and money orders payable to Immunitrition, LLC and mail your payment along with your health questionnaires to the following mailing address:
